Welcome to Two Rivers Insurance Services' Voluntary Payroll Deduction Benefits web site. The information on these pages explains the benefit and enrollment options for the 2009 Voluntary Payroll Deduction State of Iowa Employee Vision Program.
Open enrollment for 2009 begins January 1, 2009 through February 28, 2009 for an effective date of April 1, 2009. Only new hires may enroll or change their enrollment before the next open enrollment period. Thank you.
Already Enrolled?
If you have already enrolled in a vision plan in a previous year, you don't need to do anything to continue your coverage! You selections from the previous year will continue in 2009.
Options
For the 2009 plan year, the options available for you to choose from are provided by AVESIS and EyeMed. Current members will not be required to re-enroll. New members may either fill out either an online or paper application to enroll in a plan offering.
Enrollment
Open enrollment for the 2009 plan year is now closed. Paper forms are still available to facilitate qualified enrollment changes.
Two Rivers Insurance Services, AVESIS, and EyeMed sincerely hope this option to choose your plan design, rates, and network providers meets your approval. We welcome the nomination of vision providers not currently included in these networks and we have been assured that each of these nominiations will be contacted and offered a chance to consider joining the two provider networks.

THE OPEN ENROLLMENT PERIOD FOR STATE OF IOWA, UNIVERSITY OF IOWA AND IOWA DEPT. OF TRANSPORTATION EMPLOYEES FOR 2009 IS FROM JANUARY 1ST TO FEBRUARY 28TH. IN ORDER TO ASSURE A TIMELY ENROLLMENT FOR PAYROLL PROCESSING AND ISSUANCE OF MEMBERSHIP ID CARDS, NO LATE ENROLLMENTS WILL BE ACCEPTED. PAPER APPLICATIONS MUST BE POSTMARKED NO LATER THAN FEBRUARY 28, 2009.
