PLEASE DO NOT SEND OR GIVE ANY ENROLLMENT MATERIALS TO YOUR PAYROLL OR HUMAN RESOURCES DEPARTMENT. YOU MUST REMAIN IN THE INSURANCE FOR ONE CONTRACT YEAR. LEAVING THE PLAN WITHOUT AUTHORIZATION WILL RESULT IN YOUR BEING BILLED FOR THE UNCOLLECTED PREMIUMS.

Two Rivers Insurance Services, AVESIS, and EyeMed sincerely hope this option to
choose your plan design, rates, and network providers meets your approval. We
welcome the nomination of vision providers not currently included in these
networks and we have been assured that each of these nominiations will be
contacted and offered a chance to consider joining the two provider networks.
