web site. The information on these pages explains the benefit and enrollment
options for the Voluntary Payroll Deduction State of Iowa Employee Vision
Program.
Open enrollment occurs from January 1 to February 28 with an effective date of April 1 of that year. After this time, only new hires may enroll before the next open enrollment by completing an Avesis or EyeMed form. The online enrollment and paper forms and instructions are listed in the enrollment section below. If you are not making a change or cancelling your coverage then you will not need to do anything.
PLEASE DO NOT SEND OR GIVE ANY ENROLLMENT MATERIALS TO YOUR PAYROLL OR HUMAN RESOURCES DEPARTMENT. YOU MUST REMAIN IN THE INSURANCE FOR ONE CONTRACT YEAR. LEAVING THE PLAN WITHOUT AUTHORIZATION WILL RESULT IN YOUR BEING BILLED FOR THE UNCOLLECTED PREMIUMS.

Two Rivers Insurance Services, AVESIS, and EyeMed sincerely hope this option to
choose your plan design, rates, and network providers meets your approval. We
welcome the nomination of vision providers not currently included in these
networks and we have been assured that each of these nominiations will be
contacted and offered a chance to consider joining the two provider networks.
